Job Description:
A company is looking for a Concierge Care Manager to provide
care management services to members in specialized programs.
Key Responsibilities
Direct day-to-day management of high-touch member care and
treatment plans
Coordinate clinical services with external providers and community
resources
Participate in team training and prepare for audits and quality
assurance efforts
Required Qualifications, Training, and Education
Associate's degree in Nursing or equivalent qualifications in
social work or psychology
Five years of case management experience as a RN or two years in a
health insurance environment
Active, unrestricted RN license or licensure as a social worker,
psychologist, or counselor
Nationally recognized case management certification to be obtained
within two years of hiring
Knowledge of quality improvement processes and contract
language
